PROJECT AND MAINTENANCE AGREEMENT

and the DEPARTMENT OF TRANSPORTATION, hereinafter referred to as the Department, hereby agree as follows:

That the Department will construct a project for the improvement of ______ miles of the ____________________ Highway from _________________________ to ____________________ , Station _________ to Station ______ known as ___________________________________ (Local Name of Street or Road) and to be designated as Georgia Project No. _________________ , subject, however, to the condition that the County/City shall provide for its proper maintenance after completion of its improvement, and will make ample provision each year for such maintenance, and also will, prior to or during the construction of the project, and for the duration of usage for this purpose provide the following additional services, certifications, and information, at no cost to the Department.

____ 2. All utility relocations or adjustments

____ 3. Patching necessary prior to resurfacing satisfactory to District Engineer

____ 4. Clearing of ditches and drainage structures

____ 5. Leveling of shoulders subsequent to resurfacing of pavement

____ 7. Operate as a free Park & Ride Lot includinq routine security patrol.
